You must use the "iax2 provision" command within Asterisk. You will want to make the user like so in iaxprov.conf with the options you want to disable as a flag:

[si-000364000127]
user=test
pass=blah
flags += debug,disablecw

Options to use as a flag are:
disablecid
disablecw
disablecidcw
disable3way

Reload asterisk and then run "iax2 provision ip_address si-000364000127"
ip_address being the ip address of the Iaxy.
